The Dental Group at Carpenter’s Crossing

New patients

Your first visit is all about you.

Your comfort, your happiness and your health. We know a trip to the dentist can feel stressful for you or your child, and we're sensitive to that — our goal is to make you feel at home while giving you the best care possible.

New patient forms

Our new patient forms are available to complete online before your appointment. The Patient Information Form gives us your basic details — name and contact information, insurance information and a brief medical history.

Filling it out ahead of time means less paperwork in the waiting room and more time with your dentist. If you have any questions, just give us a call.

What happens at your first visit

At your first visit, we will:

  1. Review your medical and dental history forms with you.
  2. Take x-rays of your teeth and mouth.
  3. Perform a periodontal exam to make sure your gums are attached tightly to your teeth, with no gum disease or bone loss.
  4. Meet with your doctor to discuss your x-rays, answer your questions and create your own individual treatment plan.

What to bring

Bring your smile — and please remember to bring these to your first appointment:

  • A photo ID
  • Your insurance card
  • Your completed patient information form, if you didn’t fill it out online

Regular checkups keep your smile healthy

The American Dental Association recommends visiting your dentist every six months for a checkup and routine cleaning to keep your teeth and gums healthy.

We know you have a busy schedule, so please plan for your appointments to last between 45 and 90 minutes. We open at 7am Monday through Friday to make early visits easy.

New patients are always welcome.
